Image Sharing & Data Protection Consent
1. Purpose and Scope
This consent text has been prepared to protect the confidentiality of personal data such as photographs and messages submitted to us by our patients, and to explain the purpose for which they are used. At CranioWell, we attach great importance to the privacy of patient information and the protection of personal data.
2. Conditions of Use for Photographs and Data
- All photographs and messages submitted through this form will be used solely for medical assessment, analysis and patient consultation purposes.
- Shared images will under no circumstances be shared with third parties, institutions or commercial platforms and will not be used for any commercial purpose.
- Photographs and other information will be viewed only by authorised specialists.
3. Data Storage and Security
- All submitted photographs and messages will be stored in a confidential and secure system and will be used solely for patient-specific medical analysis.
- All personal data are protected in accordance with the Personal Data Protection Law No. 6698 (KVKK) and applicable legislation.
- The data retention period will be limited to the legally prescribed period after the need for medical examination and analysis has ended.
4. Patient Rights and Consent
- The patient or their guardian may withdraw their consent to the storage or analysis of photographs and messages in the system at any time.
- Patients have the right to request information about the processing of their personal data.
5. Approval and Acceptance
By submitting this form:
- You accept that your photographs and messages will be used solely for medical evaluation and consultation purposes,
- You acknowledge that your personal data will be stored securely and processed in accordance with the applicable legislation,
- You are deemed to have confirmed that you have read and accepted the terms set out in this text.
