How to Measure Baby Head Circumference
Head circumference is measured with a flexible tape measure along a horizontal line passing just above the eyebrows and around the widest point at the back of the head. Measure when baby is calm; record the largest reading.
Steps:
- Equipment: Flexible, non-stretch tape measure
- Position: Baby looking straight ahead, head still
- Landmark: Above eyebrow ridge in front – occipital protuberance at the back
- Repeat: Take 2–3 measurements and record the largest
What If Head Circumference Is Small? (Microcephaly)
If head circumference falls below the 3rd percentile (microcephaly), a medical evaluation is recommended. Microcephaly is not always pathological; however, it can be associated with conditions such as craniosynostosis (premature fusion of skull sutures).

What If Head Circumference Is Large? (Macrocephaly)
Head circumference above the 97th percentile is defined as macrocephaly. If larger head size also runs in the family, it may be benign familial macrocephaly. If rapid growth or signs of raised intracranial pressure (bulging fontanelle, vomiting) are present, urgent evaluation is needed.
How Often Is Baby Head Circumference Measured?
During routine well-child visits, head circumference is measured at the following intervals:
- 0–6 months: At every paediatric visit (typically every 1–2 months)
- 6–12 months: Every 2–3 months
- 1–3 years: Every 3–6 months
