When Should Helmet Therapy Start? Ideal Months

Golden window (4–7 months), effective period (8–12), late options to 18 months, why the first 3 months are watchful waiting, and estimated duration by age.

Babies grow almost overnight. That rapid growth is also what makes head shape remolding possible.

Parents often ask: “Should we wait a little longer, or start now?” At CranioWell Orthotics & Prosthetics Centers, timing is a biological opportunity—not only a calendar question. Here are the periods that usually matter most.

The “Golden Window” (Months 4–7)

The most efficient remolding period is often between about 4 and 7 months—sometimes called the golden window.

Why these months?

  1. Peak growth: Head circumference expands very quickly in the first half-year. Remolding works by directing that growth.
  2. Softer bones: Cranial bones remain highly malleable.
  3. Shorter courses: Starts in this window often finish in roughly 2.5–4 months when wear is consistent.

Effective Period (Months 8–12)

If months 4–6 have passed, that does not mean “too late.” Ages 8–12 months remain a useful treatment window.

Differences:

  • Growth is slower than in early infancy.
  • Babies are more mobile and may try to remove the helmet.
  • Outcome: Care can still succeed, but duration may extend (for example, around 4–5 months).

Later Period (Months 13–18)

After the first birthday, the fontanelle trends toward closure and bones stiffen.

  • Still possible? Sometimes yes, with realistic expectations.
  • Challenges: Slower growth means less remolding drive.
  • Course: Duration may approach 6 months, and change may be less dramatic than early starts. The goal is often a more acceptable asymmetry level rather than perfect symmetry.

After 18 Months?

Usually no. After about 18 months, bones are largely fused and stiff. Helmets then offer little remolding benefit and mainly add discomfort. Except for rare medical exceptions, CranioWell does not recommend starting remolding after this age.

Why Wait in Months 0–3?

Neck muscles are still weak and the scalp is sensitive. Mild early flattening often improves with repositioning and physiotherapy. The first 3 months are typically an active-observation period.

Frequently asked questions

Our doctor said “wait, it will improve,” and our baby is now 8 months with little change. Too late?

Eight months is not too late, but starting at 4 months would likely have meant a shorter course. If asymmetry remains clear, arrange an assessment soon.

How is age counted for premature babies?

Use corrected age. A chronologically 6-month-old baby born 2 months early has a corrected age of 4 months—often still inside the golden window.

Is winter better than summer for starting?

There is no seasonal medical requirement. Heat can make wear less comfortable in summer, but growth does not wait for weather. The best time is when the problem is recognized and care is indicated.

Summary

Waiting for time alone can mean missing a more responsive remolding window.

If your baby is about 4–7 months, use that biological advantage. A free measurement at CranioWell Orthotics & Prosthetics Centers can clarify current status and treatment potential. Helmet fitting is provided only at our centers. Earlier appropriate starts often mean earlier completion.
