Helmet therapy uses a custom medical device to improve plagiocephaly, brachycephaly, scaphocephaly, and related differences by guiding cranial growth in a controlled way. It is typically preferred for positional deformities caused by sleep posture or repeatedly lying with the head turned one way.

Types and Causes of Head Shape Differences

  • Plagiocephaly (flat head syndrome): Flattening on one side, often with a bulge on the opposite side. The most common positional deformity.
  • Brachycephaly (short head): Widening and shortening at the back of the head, often linked with back sleeping.
  • Scaphocephaly (long head): Narrow side to side and long front to back. Rarely positional; sometimes a clue to craniosynostosis (early suture closure).

Contributing factors include intrauterine position, difficult births, prematurity, prolonged time in one posture, and torticollis (neck muscle shortening).

Helmet Therapy Process: Step by Step

Each infant’s plan is individualized.

1. Evaluation and Diagnosis

Before therapy begins, cranial shape is assessed in detail through physical examination and, often, 3D scanning. Scanning helps define severity and type.

2. Custom Helmet Design and Fabrication

From scan data, an orthosis is designed to fit the infant’s head. Helmets are usually lightweight and breathable, with planned contact and clearance zones. Fitting is provided at CranioWell Orthotics & Prosthetics Centers.

3. Treatment Course and Wear Instructions

After fitting, wear often approaches about 23 hours a day. Consistent use is important for effectiveness. Duration depends on severity, age, and growth rate and usually lasts several months.
